A Vacancy at Norfolk Community Health and Care NHS Trust.This is a really exciting opportunity to join the Unplanned Community Response team in North Norfolk. This will predominantly involve face to face nursing assessments and care within our patient’s usual place of residence and will participate in all of the services offered by the Urgent Community Response Team, including admission avoidance, end of life crisis, and supporting the introduction of Virtual Ward service. You will also participate on clinical triage as part of the NEAT referral pathway. This is a 7 day a week service, therefore some weekend and Bank Holiday working will be expected.
