We have a fantastic opportunity for Band 6 RMN to join the Home First Enhanced Care Home Team completing Holistic assessments and completing MDT’s for care home residents. The Enhanced Care Home Team RMN will work as an autonomous practitioner providing care for patients and supporting the delivery of expert clinical care, You will work as part of an MDT and the scope of the role will be within a defined locality team as part of the community nursing service. You should possess good interpersonal and time management skills, be a good communicator, organized and able to prioritize and work well as a member of a team.
